If you believe your current services no longer meet your needs, you can request an eligibility review. This is especially important if there have been recent changes such as new diagnoses, falls, hospitalizations, or increased caregiver stress.
Contact Regency to start the review. We may ask for updated medical records, physician notes, or hospital discharge summaries. Once the information is gathered, we work with your health plan or waiver program to reassess your eligibility and hours.
